Step 1 — Release of superficial tethering
We use fine micro-needles to precisely cut the broad, shallow fibrous adhesions just under the skin. This frees the surface so the visible depressions look smoother and more natural.
Step 2 — Deep adhesion release (with cannula)
Stronger, “root-like” adhesions anchored in the deep dermis/subcutis are released with a cannula across a wider area—gentler and safer than the older Nokor technique (which often caused more bruising and pain). This step is critical for rolling and boxcar scars and helps minimize bruising and discomfort.
Step 3 — Regeneration signaling (micro-needling)
Finally, controlled micro-bleeding is induced with micro-needles to summon blood flow and growth factors, activating fibroblasts so collagen remodeling can truly begin.

What Is Subcision?
Depressed scars are often tethered from below by fibrous bands that pull the skin inward.
Subcision cuts those fibrous tethers so the skin can rise back up.

“Multi-Subcision” (3-Step Subcision + Er:YAG Shoulder-ing)
An upgraded protocol that adds shoulder-ing (edge feathering) to classic subcision.
Even when the bottom is released, steep scar edges can limit how smoothly the center fills. The skin tends to heal toward the height and texture of its surroundings, but sharp walls block that blending.
Shoulder-ing lightly feathers the scar rim with Er:YAG, creating a gentle slope so the rising center can merge seamlessly with nearby skin—often the missing piece when previous subcision didn’t meet expectations.
Who Benefits from Multi-Subcision?
- Limited improvement from prior subcision/laser
- Often the issue is not the release below, but the steep edge above. Shoulder-ing addresses that.
- Deep, sharply edged scars (e.g., boxcar)
- Softening the vertical wall helps the central area fill and blend more naturally.

How many sessions are usually recommended?

Is there any downtime?
It may be accompanied by redness, but it does not interfere with daily life. However, there are individual differences in how long it takes for the red marks to completely disappear.
What is the difference between Saesalchim and Multi-Saesalchim?
Saesalchim is a procedure that severs the fibrous tissue beneath the scar, allowing the scar base to regenerate and rise. Multi-Saesalchim is an enhanced procedure that, in addition to the effects of traditional Saesalchim, includes a process of gently refining the periphery of depressed scars. It is effective for deep scars with distinct edges and is a more comprehensive scar treatment method.
If the skin is shaved with Multi-Saesalchim shouldering, will the scar not appear larger?
Even if the edges are shaved with shouldering, the scar does not appear larger. Shouldering does not widen the scar; instead, it is a process that gently reshapes distinct edges to encourage our body to regenerate the skin to be similar to the surrounding area, thus allowing it to fill in more naturally.
